Myocarditis is inflammation of the heart manifested by chest pain, signs and symptoms of heart failure, dramatic ECG changes, elevations in a blood test indicating cardiac damage (troponin), and in some cases, imaging evidence of dysfunction of the heart. Rose and McCullough have demonstrated in the US CDC VAERS System that COVID-19 vaccine-induced myocarditis peaks in late adolescents but has a long skewed tail of cases up to age 50 and the vast majority of cases are men.
Hoeg and colleagues have demonstrated that the expected rates of myocarditis in young persons aged 12-17 could be as high as 162 cases/million persons injected. This is far greater than the background rate of 4 cases/million/year before COVID-19, as described by Arola and colleagues in Finland. Hoeg demonstrated that a young person is more likely to be hospitalized with vaccine-induced myocarditis than ever be hospitalized from COVID-19 respiratory illness.
Since COVID-19 infections are so well-tolerated by young people, even one case of myocarditis in the US or elsewhere is too many. Sadly, there are 15,424 confirmed myocarditis/pericarditis cases in the US CDC VAERS system. Fatal cases have been described in the medical literature in the US and Korea, and there will likely be more reports that become filed in case series and then cohort studies.
The explosion of sudden cardiac death among high-level athletes has drawn attention to the possibility that COVID-19 vaccination forced upon the players may induce a subclinical or perhaps denied syndrome of myocarditis that can last for months. Part of the treatment of myocarditis is rest from physical activity. The concern here is that athletes may recognize that 3-6 months away from their sport means losing a season, income, and perhaps the future. By playing through myocarditis, athletes face the real risk of sudden cardiac death as a result of ventricular arrhythmias or pump failure.
The immediate short-term approach would be to systematically screen athletes for myocarditis at time points after vaccination with cardiac troponin blood testing, ECG, and echocardiography. Without any safety measures in place, continued vaccination of athletes is likely to result in harm to some, including sudden cardiac death.
