Many people had never heard the word transgender, or if they had, they never gave it a second thought. I remember as a pre-teen hearing about Christine Jorgensen’s sex change operation in Denmark, but it didn’t really register. In medical school and residency in San Francisco, I became more deeply acquainted with the issue. But it was not all over the news or a hot topic of discussion. Now TGI (Transgender, gender non-conforming, or intersex) is part of our lexicon. 

The science says other than some chromosomal anomalies, our X and Y chromosomes determine our sex. Now there is disagreement on that long-held scientific principle. Nearly 0.6% of U.S. adults identify themselves as transgender, which is an incongruence between the physical sex characteristics and gender identity. Understandably, this can lead to mental distress.  

In an attempt to reconcile their feelings, some people have chosen to turn to medical and surgical means to change their bodies. When it comes to adults, they can decide what they want to do to their bodies as long as it does not harm others. Most adults can understand the many systemic adverse effects of pharmacological treatment for patients who have transitioned or are in the process of transitioning to the opposite sex. These may include an increased cardiovascular risk in both transwomen and transmen, a significant increase in body mass index and systolic and diastolic blood pressure in transmen and osteoporosis, venous thromboembolism, fractures, cardiovascular disease, stroke, and hormone-dependent cancers.

Some have decided that the benefits of transitioning outweigh the risks. 

The real conflict arises when it comes to children. Children are being encouraged to explore their gender identity as young as kindergarten. Several weeks ago, I had a guest who discussed sex change and hormonal treatments in children as a form of child abuse — and bad medical practice. Indeed, many countries have banned such therapies for youths and favor focusing on support and psychological care. This episode will discuss the transgender issue from a different point of view. 

P.S. I waited until Gay Pride month was over since the gender issue should not be a political issue. It is a medical issue to be explored and debated, ensuring that physicians will “do no harm” to their patients.
